Appointment Check-In

All patients MUST present their ID and Insurance Card to the receptionist as well as update their contact information (e.g., Address, Phone Number, etc.) when CHECKING-IN for ALL APPOINTMENTS.

Confirming and updating your insurance information is imperative so that we can continue to provide you with the best care possible as well as to avoid any billing issues.

Health Insurance

If you have any problems with your health insurance carrier, please contact your health insurance directly at the number located on the back of your health insurance card.

If you have any questions regarding eligibility and whether Trinity Family Medical Clinic is an in-network provider, please call your health insurance carrier directly at the number on the back of your health insurance card.

New Patients

New Patient Requests

Please fill out our New Patient Request Form to inquire about your eligibility to see one of our providers.

NOTE: Please do not include any confidential or sensitive health information in your message. Only include general information and we will contact you to discuss specifics.

New Patient Request Process

Everyone who wants to become a new patient of Trinity Family Medical Clinic will go through the following application process:

  1. Apply by submitting the New Patient Request Form. (NOTE: OUR PROVIDERS DO NOT ACCEPT PAIN MANAGEMENT PATIENTS AND MEDICAID/APPLE HEALTH)

  2. After submitting the request form, our medical staff will review your information.

  3. Once your information is reviewed, the receptionist will give you a call to either schedule an appointment or inform you that our providers will not be able to take you on as a patient.

  4. If you schedule an appointment, please note that on the day of your appointment, our providers will get to know you and assess whether it is truly the right fit for them to be your primary care provider. If so, you have officially become a patient of Trinity Family Medical Clinic.

For your first appointment, please bring your ID and insurance card with you to present to the receptionist. Also, please plan to arrive 15-20 minutes prior to your appointment to input your information into our EHR system and to fill out our Welcome Packet.

Established Patients

Scheduling Appointments

Appointments for ESTABLISHED PATIENTS can be scheduled over the phone, in-person, or via MyChart. NO NEW PATIENT APPOINTMENTS MAY BE SCHEDULED VIA MYCHART. Any new patient appointments scheduled via MyChart will be subject to cancellation.

Please Note: Any appointments made may be rescheduled or cancelled due to scheduling conflicts, emergencies, or provider absences. Patients will be contacted for all rescheduled and cancelled appointments.

Patients Transferring to TFMC

If you are a previous patient of our providers, but have not seen them in over 2 years or if it is your first time seeing one of our providers, you are considered as a new patient and MUST follow the New Patient Request Process.

Prescription Refill Requests

Prescription refill requests are preferred to be done through your pharmacy, however, requests can also be made in-person or over the phone. This is to ensure that all your information is kept confidential in accordance with HIPAA regulations and allows the medical assistants and our providers to confirm whether the prescription can be refilled or if you need to schedule a follow-up appointment.

Medical Records Request

If you require a copy of your complete medical records for any reason, please fill out the Medical Release Authorization form below. You can return the form to us by via email, at the clinic, or ask your new provider’s office to fax a request to 253.881.1925.

Multicare MyChart Patient Portal

If you are a current patient at TFMC and are in need of an appointment you can schedule one by either calling the clinic at 253.881.1844 or by logging into your MyChart patient portal.

Through MyChart you can schedule doctor's appointments, request prescription renewals, review your health history, and more. If you are using MyChart for the first time, you can request a MyChart activation code by filling out a form online or call the clinic at 253.881.1844 to request a personal access code.
